Psychiatrist supervised flow tDCS (transcranial Direct Current Stimulation) treatment

Transcranial Direct Current Stimulation is a type of neuromodulation treatment which is NICE accredited for treating Depression. In this treatment, a gentle 2mA electric current is delivered through a headset to stimulate the part of the brain which is known to be responsible for emotional regulation, and is hypoactive in people with depression. This device is safe and available for purchase over the counter for patients to use at home independently in the UK. However, patients find it beneficial when this treatment is conducted under ongoing supervision, monitoring and guidance from their Consultant Psychiatrist. Weekly supervision can be done through a secure online platform and regular follow-up appointments are done to regularly review treatment plans.
ADHD in Adults

Attention Deficit Hyperactivity Disorder is a developmental disorder of childhood which can often persist into adulthood. It can manifest itself by causing problems in one's ability to focus, pay attention to detail and complete tasks, organise oneself or remain patient in situations which demand patience. Dr Desai-Gupta offers Psychogenetic testing to patients, where relevant, to identify the Psychotropic medication which is most suited to your genetic make-up, so as to maximise effectiveness and minimise side-effects.

Stages of Treatment for Deep rTMS Therapy

Suitability Assessment

If referred to Atrom Mindcare for Deep rTMS therapy you will initially have a 60-90minute assessment with our Consultant Psychiatrist*. He/She will ask you questions about your illness to confirm your diagnosis and conduct a risk assessment for your suitability for treatment with Deep rTMS. He/she will ensure that the treatment plan is tailored to your brain. She will then discuss the treatment process, your individual risks, effects, side-effects of treatment in detail with you and answer any questions that you may have before progressing onto treatment sessions.

If you are already seeing our Consultant Psychiatrist for your mental health problem and it is felt that Deep rTMS is the appropriate treatment for you, the treatment will be discussed with you and a risk assessment for rTMS will be conducted during your General Psychiatric Assessment or Follow up appointment.

The Deep rTMS Treatment may be right for you if :

you have a current diagnosis of depression (with or without anxiety symptoms).
you have been on antidepressant medications during the current depressive episode without sufficient benefit or with intolerable side effects.
you have attempted psychotherapy during the current depressive episode without sufficient benefit.
you have no history of or recent episodes of seizures, no implanted metal or electronic devices in or around thehead (dental work is fine).

* If you are referred to Atrom Mindcare by your Consultant Psychiatrist, he/she will be responsible for your overall Mental Health care, and they will have confirmed your diagnosis and assessed your suitability for TMS. We will require a letter/ report from them to confirm this. Consultation with our Consultant Psychiatrist will, in this case be of 30mins duration and entail a risk assessment and a TMS prescription tailored to your needs.

Treatment Course for Depression

Deep TMS sessions for depression last approximately 20 minutes, as opposed to 40-minute standard rTMS sessions. Alternatively, a 3-min intermittent theta burst (iTBS) session is also available. A Deep TMS treatment plan usually requires daily sessions over four weeks, followed by periodic maintenance sessions.

Treatment Course for Obsessive Compulsive Disorder

Deep TMS sessions for OCD treatment involve a brief personalized provocation, or exposure, to activate the relevant areas of the brain, followed by an 18-minute TMS treatment. A full course of Deep rTMS OCD treausually requires daily sessions over a six-week period.

What to expect during treatment sessions

You will be greeted in the reception area by our friendly technician who will lead you to the treatment room. He/ She will explain the process to you once again.
You will need to remove any hearing aids, hairgrips, jewellery including necklaces, earrings and face or tongue studs and anything else containing metal
You can wear your usual clothes and won't need to get changed
You can take your phone into the treatment room, but it will need to be put to one side during treatment
Please have something to eat and drink before your appointment. You will be offered tea/coffee whilst you wait in the reception area.
There will be plenty of time to use the bathroom before your treatment begins
Each treatment session will take about 20-30min, however your first appointment will be longer as the dosage of your treatment will be set at this appointment. If it's going to be any different (depending on the type of TMS you are having), we'll discuss this with you
If you are travelling by car, please allow time to park. You will be sent details about parking by our team ahead of your appointment.

You are welcome to bring one family member or friend along if you wish, to the clinic. If more than one person attend, we may ask them to wait in the reception area as the treatment room is small. Equally, you can attend the treatment alone, our technician will be there with you during the treatment to provide any help or support that you may need.
At the beginning of each treatment, we'll run through a safety checklist with you.
You'll be seated in a comfortable chair and our nurse will place a magnetic coil device on your scalp which is like a tall hat.
The initial session is approximately 30 minutes.
Motor Threshold Mapping: the coil, that is located inside the helmet, is calibrated to the appropriate magnetic field intensity and optimal treatment location based on the response of your right hand to single pulses over the motor cortex. The mapping process is repeated periodically throughout the course of treatment to ensure proper calibration of the coil.
Deep rTMS at Atrom Mindcare is operated by a trained, certified professional who follow strict guidelines
to ensure patient safety.
There will be several short pulses, with a gap in between. As the magnetic pulses are delivered, you will hear clicking sounds and feel a tapping sensation. If this is uncomfortable, please let us know. We can provide you with ear plugs and adjust the coil slightly.
You can read or listen to music during your treatment.
Our TMS-technician will provide your treatment. They will stay with you all the time and can answer any questions or worries you have.
If you'd like to pause or stop the treatment at any point, please just ask.

Think of daily treatment sessions like going to the Gym for your Brain.
Deep TMS for Major Depressive Disorder consists of 20 sessions across a 4-week period followed by periodic maintenance sessions.
Each session typically takes 20 minutes involving stimulation of brain targets with Deep rTMS. The treatment coil delivers gentle electromagnetic pulses to key areas in your brain involved in mood regulation. These pulses feel like mild tapping on your scalp.
You’ll be comfortably sitting on a chair, awake and alert during your treatment.
This means you won't need any recovery time and can drive and carry on with your usual daily activities immediately after your treatment, such as going to work or shopping.
To help us assess your progress, we may ask you to complete a questionnaire at the end of your session.
You'll then come back the next day (Monday to Friday) until the end of your treatment.
We will update your GP and referrer weekly regarding your treatment progress, if you have given us permission to contact them.

After your last treatment session, a range of mood rating scales will be completed by your Technician or Consultant. You'll also be asked to complete a questionnaire and provide feedback online.
You will have a choice to attend a review meeting face to face or online with your Consultant, which we recommend, which will last 15 to 30 minutes. This will focus on the results from the mood scales, questionnaire, and feedback, which will inform whether further treatment is needed and recommendations.
If it's agreed you don't need further treatment, you'll be discharged, and a letter will be sent to you and your GP.

SIDE EFFECTS

For Depression, the most common side effects observed in the Brainways multi-center randomized clinical trial during treatment include:

Site pain (25%).
Site discomfort (19%).
Jaw pain (10.2%).
The following side effects were not significantly different in the treatment vs. sham (placebo) groups, and therefore may not be specifically caused by Deep TMS treatment:
Headaches (47% in treatment group, 36% in sham group). Such headaches tend to get better or go away completely with successive treatments, and can be alleviated using over-the-counter medication. We will offer you Paracetamol if you say you are experiencing a headache or any aches and pains after your treatment.
Muscle twitching (6% in treatment group, 1% in sham group).
Back pain (4% in treatment group, 8% in sham group).
Anxiety (5% in treatment group, 7% in sham group).
Insomnia (7% in treatment group, 7% in sham group)
For Obsessive Compulsive Disorder, the Brainsway prospective, double-blind, randomized, controlled, multi-center trial on Deep TMS effects on patients with Obsessive-Compulsive Disorder (OCD), the following side effects were observed. (No significant differences were seen between the treatment and sham groups, suggesting that these side effects may not be specifically attributable to the magnetic stimulation of the brain involved in Deep TMS treatment.)
Headaches (32% in treatment group, 35% in sham group).
Anxiety (12% in treatment group, 3% in sham group).
Jaw pain (8% in treatment group, 1% in sham group).
Common cold (8% in treatment group, 9% in sham group).

( Inpatient Treatment if required at Priory Hospital in North London under Dr Desai-Gupta We offer Face-to-Face Psychiatric appointments to patients based in London, Hertfordshire and Essex at Atrom Mindcare Clinic in Epping. We also provide online Consultation to patients from all over United Kingdom. Prescriptions can be emailed securely to you or your GP. If you are based far from London and therapy or inpatient treatment is needed, we liaise with your local services to facilitate this. Dr Desai-Gupta, Clinical Director at Atrom Mindcare has admitting rights at The Priory Hospital in North London and if needed, she is able to organise inpatient care for you here.
Psychiatrist supervised flow tDCS (transcranial Direct Current Stimulation) treatment

Transcranial Direct Current Stimulation is a type of neuromodulation treatment which is NICE accredited for treating Depression. In this treatment, a gentle 2mA electric current is delivered through a headset to stimulate the part of the brain which is known to be responsible for emotional regulation, and is hypoactive in people with depression. This device is safe and available for purchase over the counter for patients to use at home independently in the UK. However, patients find it beneficial when this treatment is conducted under ongoing supervision, monitoring and guidance from their Consultant Psychiatrist. Weekly supervision can be done through a secure online platform and regular follow-up appointments are done to regularly review treatment plans.

Attention Deficit Hyperactivity Disorder is a developmental disorder of childhood which can often persist into adulthood. It can manifest itself by causing problems in one's ability to focus, pay attention to detail and complete tasks, organise oneself or remain patient in situations which demand patience. Following a comprehensive assessment and risk assessment and a detailed history regarding substance misuse, in some patients where it is felt that Medical Cannabis would be beneficial to their mental health and the benefits outweigh the risks, Dr Desai-Gupta is able to facilitate prescriptions for Medical Cannabis. Dr Desai-Gupta offers Psychogenetic testing to patients, where relevant, to identify the Psychotropic medication which is most suited to your genetic make-up, so as to maximise effectiveness and minimise side-effects.
